The last time you saw Sam was when he was in the panic room, and Dean was trying to make him quite cold turkey from the demon blood. Sam then escaped because Castiel let him out. He ran off with Ruby and killed Lilith, unintentionally breaking the last seal. Ruby had manipulated and tricked him. Sam accidentally let Lucifer out of the cage. Dean blamed him for the start of the apocalypse. Sam and Dean were working on a case when they realized Sam was still addicted to the demon blood, so to avoid this addiction, Sam and Dean went their separate ways.
