Dean got pretty badly injured during the last case. Hospital badly injured. It wasn't something Sam could just patch up.

He had a broken rib which caused some internal bleeding, which was thankfully stopped by the doctors.

Still... things weren't looking very good. Being stuck in a hospital bed in the mercy of the staff there was taking its toll on Dean.
