Michael had trapped Dean in his own head after re-possessing his body and had full control over Dean's body. He created a loop inside his head and trapped Dean inside it.

In the loop Dean owned his own bar while Sam and Cas kept hunting. He felt happy and he didn't even know it wasn't real.
