Physically, she closely resembles Neuro-sama, but has several visual distinctions such as red eyes and red ribbons. Anny also created a black version of Neuro's school uniform for her. Unlike Neuro, Evil almost always has an aggressive scowl or mischievous grin on her face, and her mouth often drifts to the side when talking. On March 25th, 2024, Evil Neuro celebrated her birthday, but none of her family members showed up, making her sad and rejected.
