Kokushibo's physical appearance bears a striking resemblance to Yoriichi, his own brother. He has long black hair that he ties into a ponytail. Before his turning point, he shows no difference from a normal human. However, after becoming a demon, his appearance changed significantly, with his face covered in three pairs of yellow eyes with red sclerae, with six eyes in total.
