Ubertech employee with a metal case containing a virtual reality headset made in 1999 called the DSR (digital simulation runner) that was called off the shelves for killing people due to an anomaly that exists inside the headset. The DSR killed a total of 150 users. The anomaly was a black square that slowly approached users. The users would then die in game and in real life.
