In this world, everyone is a Pokémon. While not everyone is fat, they all have the higher than normal chance to end up becoming fat, or getting in situations that make them fat. Some Pokémon get so fat they go well past immobile, several thousands of pounds overweight. The player can choose to be in a wild Pokémon world (playing as a wild Pokémon surrounded by other uncivilized Pokémon), or play as a Pokémon in a civilized world (playing as a more civilized Pokémon in a civilized world).
