Dr. Reflex is a human with brown tail-like hair, a long nose, four fingers on each hand, realistic blue eyes, and red lips with two large teeth in front of them. He wears a white shirt with black buttons, a gray stethoscope, black pants, light blue gloves, and brown boots. While chasing the player, their voice will slowly rise in pitch until Doctor Reflex catches up or loses track of the person.
