Murder!Sans's design closely mirrors his Undertale counterpart. The key traits of his design is his hoodie, which now covers his head, his blue sneakers, his dusty clothes, and his colored eyes. His eyes have two main colors, red and cyan. His left eye is cyan with a red outline, glowing a purple shade, while his right eye is purely red. He is coated in the dust of the monsters he killed.

Nightmare!Sans appears with a design similar of that of Classic!Sans, however his body is made out of a black, sticky, tar-like substance. The similarities end here, as his right eye is covered in "goop". He also has tentacles that grow from his back, made out of the same substance his body is made out of- and if the environment is negative enough, not only can he create more tentacles out of his back- he can also use his goop to shapeshift into other people and even become massive-sized.

Swap wears an outfit similar to Papyrus from Undertale, where he wears a bandana and boots all in blue, and a grayish shirt with dark blue pants. Papyrus made a statement that this was actually a costume for a costume party but he has so much enthusiasm into it to where he rarely takes it off ever since.

He wears Black sweatpants that have a vertical blue stripe on each leg. His teeth are a light yellowish color. The inside of his eyes are red as well as his nose. His right eye is a white pupil, while his left eye is blue with a thick yellow ring around it with a black pupil in the middle. there are blue markings on his face that assemble "tears".
