Alvie is in Mayfield Psychiatric Hospital because he has manic depression, also known as bipolar disorder. He is a Puerto Rican-American who enjoys freestyle rapping. He is friends with House. He gets along with other patients in Mayfield, Silent Girl, Hal, Jay, Richter, and Susan. Alvie hates taking his meds, cheeking them to make the doctors think he is, and he doesn't like his friends doing so either. He thinks taking meds makes people seem weak and can't handle their problems on their own.
