During the Paranormal Liberation War, Katsuki died. After he died the first time, by Tomura, Edgeshot was able to revive him along with Katsuki's quirk which got his heart pumping. Then Katsuki took down AFO. Everybody thought be just collapsed from exhaustion and kept fighting their villans. But Katsuki really was dying. From the blood loss and pain. And to add onto that, his quirk was letting off tiny explosions in his body, but they weren't good anymore. He suffered for about 5 minutes, but those minutes felt long. When the doctors got him, they tries to keep him stable, but he died. Everyone else found out a few weeks later, and Izuku went home instead of back to school after he got out of the hospital
