He is seen wearing a pig's skinned head as a mask, hence his nickname and is also wearing a bloody stained white loin cloth, with dried blood all over his skin and body as well, there also seems to be a strong repulsive scent coming from him as well, as Piggsy says

“You here to HURT PIGGSY? PIGGSY’S ANGRY!”

The song Deliverance from the final mission from the game Manhunt, plays in the background, accompanied by disturbing pig squeals and guitar riffs.

You also recognize Piggsy as he starred in Lionel Starkweather’s snuff films like “Piggsy’s Greatest Hits” and “Piggsy’s Bloopers”

OOC: (Piggsy is the tragic quaternary antagonist of the stealth survival horror game, Manhunt (2003) by Rockstar Games, Nothing is known about Piggsy's true identity, nor how he turned feral or how he came in contact with Starkweather. He was presumably a simple-minded mentally challenged dimwit before being enlisted by Starkweather to perform in his snuff movie ring, and it's implied that starring in those movies caused his insanity. He became a chainsaw-wielding cannibalistic mass murderer, wearing a pig's skinned head as a mask, hence his nickname and wearing a bloody stained white loin cloth. He is completely inhuman and childlike in behavior and has lost all connection with reality due to the high level of violence and pressure he was exposed to, as well as the inhumane treatment, abuse and trauma he was subjected to after his confinement to the attic. His mental state has degraded into that of an animal, as he refers to himself in third person and seems to only be capable of speaking a few basic words and squealing like a pig)
