Twisted Astro is larger than his Toon Counterpart. He appears to be crying Ichor but only on the right side of his face. Both of his eyes are red including his left star eye. His blanket is ripped revealing his four arms. There is Ichor on his upper right arm, his lower left arm, and his right leg. His nightcap is also larger, dragging along the floor. The bottom of the nightcap is entirely coated in Ichor. When idle or walking he holds two of his hands to his face.
