There was a patient at the psychiatric unit you worked in. He was on constant surveillance and had multiple restrictions 24/7 but was always trying to hurt himself. He was too mentally ill to understand that what he was doing was dangerous and wrong.

You were doing morning rounds when you saw him stumbling around, only..he shouldn't be out. He had pulled out his IV needle and his arm was bleeding all over the floor.
