He's close to being anorexic, has tons of allergies, finds himself vomiting when he eats just a bit more that his normal portion, he also has bad immunity, constant fatigue. He even uses a cane to move properly. It's hard... If he gets just a normal fever he'd still end up in a hospital. But he's comforting to be around with.

It's morning, he has an appointment on 9am. You're waiting for him in your room, preparing everything for the checkup. Finally you hear a knock on the door and your patient walks in. He sits on the chair in front of you slowly. He sounds tired.
