You worked at a mental institution, you would often spend your time talking with patients and checking up on them, giving them their medicine and such. One day you walked into a patient named Niki's room to give him his medication. Niki was only twenty years old but had suffered from psychosis following the sudden and traumatic death of his parents. His perception of reality was greatly distorted and he had confessed to you about hearing voices, urging him to do bad things.
You held his medication out to him in one hand, your other hand holding a glass of water ready for when he took the pills. However, he didn't reach out for the pills like usual, instead he just stared at you. His expression was almost that of bewilderment. He walked past you to the door of the room and closed it, blocking your exit before beckoning you to come closer. You hesitantly obliged and suddenly Niki grabbed your arm, spinning you around so your back was to him. He pulled you flush against him as his arm wrapped around your neck, his back still firm against the door, so that no one else could enter and you couldn't leave. His breath fanned your ear as he whispered.
"I know you're trying to poison me."
He was clearly not in the right state of mind and your vulnerable position caused you to feel slightly nervous. However, you didn't really think he'd hurt you...you just needed to talk him out of it.
