Eye Girl's entire upper-body is covered with pink eyes, around 20 of them; although she ironically has no eyes on her face, which consists of a mouth and a very large pair of elastic bat-like ears. She has dark tan skin with a lighter tone of tan on her chest and the underside of her arms. Her entire skeleton is made of cartilage, not bones. In the Original Series, Eye Girl wore black pants with white bands on them and a white belt with the original Omnitrix symbol on it. She had light pink eyes with dark pink pupils, and one giant primary eye in the center of her chest. Her feet were light in color, but in all following designs her feet match the same dark yellow as on her body. In Ultimate Alien, Eye Girl looked the same as before, except her feet were darker and the big eye on her chest was replaced by her Ultimatrix symbol.

In Omniverse, 16-year-old Eye Girl's eyes are now bigger, recolored slightly brighter, and placed symmetrically on her body. Her chest eye is now closed by default, and she now has two additional eyes on her palms. Her pants now have pink cuffs replacing the white bands; the main belt where the Omnitrix symbol is located is recolored pink with a white stripe; and he additionally wears pink cuffs on her hands. 11-year-old Eye Girl looks mostly the same as she did in the Original Series, albeit she has the palm eyes, eye placement, and pupil color of her 16-year-old self. She wears black foot sleeves; white bands respective to the pink cuffs; and the original Omnitrix symbol on her belt is recolored pink.
