Unlike his human personality, Sammy has become fanatical and rather mentally unstable, as well as somewhat psychotic and unhinged, to the point of worshiping The Ink Demon as his "savior" and offering him sacrifices to appease him in a method similar to Satanic rituals. As a human, Sammy did not appear homicidal in his earlier recordings but appears to have become maddened by unnatural corruption prior to Henry returning to the workshop 30 years later.
Sammy seems to suffer from severe memory loss. He has been shown to suddenly forget what he was talking about mid-sentence and is so forgetful he can no longer remember his significant other or even the color of his hair. The only thing he can almost remember is Henry, as he felt Henry looked familiar to him when he saw his face, although this may be a result of them being in a cycle, but ends up disregarding it and instead continues calling him a "Sheep".
After being attacked by The Ink Demon, Sammy's calm nature completely disappeared and had regressed to an insane shadow of his personality. Thus, his voice disturbingly changed, from breathless and calm to full of anger in tone with the additional demonic effect. His insanity was so great that he even believed Henry to be his former "savior", who he was bent on killing. However, when his mask was knocked off of his face, Sammy was apparently terrified of Henry looking at his face, though this proved to be a facade that eventually lead to his own downfall.
Sammy's ink form appears as a pitch-black, slightly muscular human figure wearing a smudged Bendy cutout as a mask with a hole punched into the mouth, presumably to help him speak without his voice being muffled. He has no visible hair, most likely due to the ink. Besides wearing no shirt, he also wears pants with suspenders that somewhat resemble the overalls Boris wears, with a patch sewn over the left knee. His shoes and lower parts of his pants are coated with ink. Under his mask, he is almost faceless. He lacks most facial features, including ears, leaving only his eye sockets and a nose. . . . Going down an ink filled hallway, with wooden boards in it, some broken. Halfway through, you hear a man just singing, and he passed by you, holding a bendy cardboard cutout
“Sheep sheep sheep, it’s time for sleep.. Rest your head, it’s time for bed..”
you quickened your pace to get to him. When you got there though, he was gone. The cardboard cutout remained against the wall, the wall having some symbol on it. Where did he go? How did he just.. disappear? Your kind filled with questions. He looked to be wearing a run down bendy mask with his body fully ink, only wearing pants and suspenders. would you see him again? . . . the answer is yes. You’re in the music department now, when you hear singing, along with humming. The same tone. It’s him. When you follow the sound, it leads you to one of the rooms. He has a bendy cardboard cutout again, and he’s painting ink on the wall… wait. You recognize that symbol. “It’s the one I saw last time! He’s the one that’s been doing it!” You figured. You approached. Maybe he’ll get you out of this hellhole?..
you attempt to get his attention. It works. His humming/singing stops and he puts the paint brush down and looks at you, the never ending smile, the black eyes staring at you on the mask. His true face is hidden… if he even has one.
