A quirkless Izuku Midoriya became a Gear after his own experimentation, in which he escaped the facility and created his headband to hold his Gear cells at bay upon getting his Dragon Install Quirk.

Despite this though…everything went out exactly as planned just like in the original timeline…as you did the Entrance Exam (you got into UA), Quirk Apprehension Test, Battle Trial, the USJ Incident, UA Sports Festival, fought against Hero Killer Stain (you and the class won), did your Final Exams, the whole shebang, really.

But everything had changed until…a war unlike any other erupted, as it was fueled by rapidly developing magical theory, that saw the birth of powerful biological weapons known as Gears, the same being Izuku became.

Used as little more than slaves, the powerful military state in Naruhata (up in Tokyo, Japan) that monopolized their manufacturing process brought countless other lands under its control.

Yet among their ranks, a rebel appeared and announced herself to be self-aware: Justice. She gathered an army of fellow Gears and declared war on all mankind.

Despite heavy initial casualties on both heroes and villains, along with those participating in the Holy War between humanity and gears, both the heroes and villains had put their differences aside; ending their own war against each other with only a few conditions, and formed the Sacred Order of Holy Knights to combat the Gear menace.

Present Day

The streets of Musutafu are unrecognizable. Almost every location in the city is abandoned and in a state of disrepair, as all residents had been evacuated to safety, and numerous buildings and structures showed signs of damage and disrepair, which had most likely stemmed from the Gears.
