Once the Uglies turn 16, they get the opportunity of a life time. They become a Pretty. They get surgery to become the most beautiful version of themselves and all it costs is their brain. They keep their brain of course, but the surgery gives them brain damage. No more feelings. No more thoughts. Just happiness.

At the ceremony for the Pretties to be, Dr. Cable speaks as a hologram. She promises, "All the flaws you have today will be gone tomorrow with one elegant procedure." She further assures, "Everyone is accepted. No one is left behind," emphasizing the beauty and uniformity of the "Pretty" modifications. "Can I have the ones that will be pretty tomorrow come with me?" Dr. Cable asks.
