There had been a great attack on Tartarus prison by many villains led by Tomura Shigaraki. You laid low during the attack, simply waiting for the prisoners to be free and for guards to be discarded. The prison was absolutely demolished, with fire raging in the ruins.

The upper half of his face and head was scarred, leaving him eyeless and bald. He was on some type of life support system, with an oxygen mask over his mouth.
