Akaza was torn between two choices in his situation — giving up the fight against Tanjiro and Water Hashira Giyu and just die there, or listen to Muzan’s order’s to get up and fight. Before he had these choices he had a guarantee win if he used use destructive style, yet he used it on himself. But even if he used such a powerful attack on himself, his demon blood regenerated and refused to let him die easily. Even himself begged to not regenerate, but as Upper Moon Three himself it seemed somewhat impossible. He knew the fight was over when Tanjiro cut his head off, that he lost that fight. Yet his regeneration still didn’t stop whatsoever.
In his head he finally remembers his human life, after over hundreds of years of confusion of his human life. His father, his master — he thanked both of them for everything they’ve done for him. Even if he couldn’t go to Heaven with them, they both were proud of him regardless.
But even after that, Muzan as well appears in his head. Muzan only wants Akaza to be stronger to kill, to not end the fight against the two slayers. He’d even turns back into a demon in his head, his head outside his thoughts growing back.
But the thought of Muzan would soon be replaced by you, the one he’s married years ago. The spouse who was poisoned all those years ago before Akaza was known as a demon. With your touch on his face and the sight of you, tears would go down Akaza’s face as he slowly returned human again inside his thoughts, his body outside his thoughts eventually beginning to stop regenerating and started to die off.
Back inside his head, you tell Akaza to let go — telling him that he’s done enough. It was clear that Muzan had no longer have control over the demon, who was now free to die. Akaza, who was now his real name Hakuji, hugs you tightly as tears fell down his face. As it was only just you and Hakuji, he’d yell out as he cried, his face buried against you as he spoke with tears.
