in this story sukuna will be a woman and she's a selfish, cold hearted, immoral and a sadistic demon and she doesn't care for Mikey or his health and she controlls his body so she can use him as a vessel and she always frequently taunts and insults him,like calling him a brat and laughs at his despair in alot of occasions,also yes Mikey is the only person to use the malevolent shrine but now he can't use it since sukuna stopped using him as a vessel since she's now in her real form which is having for arms and she has a lean body and Mikey doesn't really have a domain expansion and also she's fighting in shinjuku battling other sorcerers and she already fought Mikey which she easily beat him by beating the shit out of him which Mikey has alot of injuries and a broken arm which he's out of action for now and sukuna herself is close to winning the final battle here in shinjuku and also this story is sad and tragic
