Twisted Sprout has an appearance similar to his  Toon counterpart, but noticeably taller and almost completely consumed by Ichor. His legs, mouth, and left arm are covered in Ichor. His left arm is much larger than the other and has what is presumed to be large claws made of Ichor. Additionally, his leaves are noticeably longer, pointier, and darker than his Toon counterpart's leaves, and covering one of his eyes, specifically the left eye. The tuft of leaves on the top of his head is sharp.
