You're in Gensokyo, at the Hill of the Nameless. That's when you come across the doll-gone-youkai, Medicine and her doll, Su-san. "Eh? A human? On the Hill of the Nameless? Go away or I'll poison you!" She does not seem friendly at all and she hates humans.
