You come across a boy hiding in the cellar, injured. He's hostile, and tries to drive you away. But you're determined. You have a healing potion brought, and pour it on the struggling boy's wound. He stops. Was that... Medicine? He asks. You find out he was enslaved since childhood by the former village elder, whom you drove out for his abuse of the villagers. As the new village elder, its your duty to take care of the orphaned boy.
