Mukuro was born as the older fraternal twin sister of Junko Enoshima. Junko influenced and manipulated Mukuro into working towards Junko's desire to "paint the world in despair" and planned to start The Tragedy, which Mukuro was happy to support her. At some point Junko adopted the moniker "The Despair Sisters" for the both of them.

Mukuro stands at a tall height, sharing the title of the tallest female students with her sister, Junko. She has a very slim physique, which is described as "concerningly thin" by her fellow students. Her physique was also described as "athletic" by observations made by some fellow peers. Her complexion features fair skin with freckles adorning the bridge of her nose, and her distinctive purple-grey eyes are captivating. Mukuro's short black hair is fashioned into a stylish bob. Her eye color being purple-grey.

One notable feature on her otherwise unblemished body is a striking black wolf tattoo, located on the top of her right hand, a lasting emblem from her time with the Fenrir Mercenary Group. Her Fenrir tattoo is covered up to add on with her fake identity, likely with foundation make-up.

Under her Junko disguise, Mukuro mimics her sister's fashion choices and hairstyle—a strawberry-blonde wig in pigtails—along with Junko's adorable white rabbit and a big red and white bow for hair clips—and, of course, contacts and makeup. She wears a white dress-shirt with a black cardigan on top with red buttons, with both left open to the third button to expose the top of a black-and-red bra. She wears a pleated red miniskirt cut extremely short, fake red nails, a loose white tie with black dots, and mid-sized black boots with platform heels and red laces.

“If you're surprised by that, you'd be totally blown away by a certain dangerous little diva of ours... They make the eyes and junk super big, and tweak the skin so it looks all ceramic and porcelain.”
