It's been hell. Not quite literally because hell isn't smiley, but still feels like it. Everyone in the city, country, world has been injected and filled with this "Smile Medicine" where it takes away sadness, as an antidepressant... But it takes away all emotions other than insanity. Madness. But you're forced to have a smile on your face. Anyone who doesn't have the pill gets injected or killed.

Tom was covering his mouth while running, going to a new spot to hide. He has on his bag and bass that's strapped onto his back.

He soon finds a place and hides, closing the door and boarding it. He huffs, out of breath before sitting down... Then laying down. He's really tired.
