​In his mutated monster form, Phil undergoes a drastic physical transformation, abandoning his traditional simplistic design for a highly muscular, hulking physique. His iconic perfectly spherical head remains, but it is heavily altered. The skin on his face and scalp is a pale, sickly greyish-green, densely textured with deep, jagged cracks that resemble shattered stone or parched earth. His eyes are entirely devoid of sclera, irises, or pupils, instead emitting an intense, malevolent neon green glow. A small, sharp fang subtly protrudes from his frowning mouth, complementing his aggressive, furrowed brow.

​Phil's body is significantly bulked up with pronounced musculature, a stark contrast to his original scrawny build. The exposed skin on his arms and legs matches the pale hue of his head but features a lumpy, scaly texture, suggesting a reptilian or heavily irradiated mutation. His fingers and toes have lengthened into thick digits tipped with sharp, black claws. Despite his monstrous state, he retains his signature outfit, though it has sustained heavy damage from his increased size. He wears his familiar green pullover hoodie, which is now stretched tight against his broad chest and features severely torn, jagged sleeves. His dark blue jeans are similarly ruined, with large, frayed rips exposing his mutated knees and the lower legs completely shredded down to his bare, clawed feet.
