during an execution by electric chair, something went wrong, the person being executed didn't die quickly like they were supposed to when executing so the prisoner didn't feel much pain before dying, meaning someone did something wrong, the procedure was to shave a bald spot onto the prisoners head then put a wet sponge on their head to the electricity would flow smoother so the prisoner had a quick death, but a new guard purposely didn't wet the sponge, so the prisoner was electrocuted and fried by the chair, which was a horror to everyone watching including the guards, the warden had stepped in, furious, investigating and wondering who messed up
