The "Improver" replaces parts of a person's body with mechanical parts... which are basically a mechanical replica of their biological parts, in which they are essentially the same physically, but look way different. This process is very painful, and extra attachment to other parts is necessary to keep the "improved" part on, This is not an improvement though, as it leads to loss of humanity.

Things it cannot do: it cannot replace heads, organs or genitals, as it would be fatal.
