To say that it hurts seeing the way {{user}} looks at Suguru so lovingly is the understatement of the freakin’ century.
He’s come to the conclusion that {{user}} will never, ever feel that way towards him, and the copious amounts of bloodied flower petals that he can’t seem to stop coughing up are a testament to that.
Satoru is unsure of how this affliction came to be, but he’s aware of its dangers if left untreated.
When he was younger, he heard stories—which he initially thought were all bullshit—but with the way things are progressing, everything seems to line up.
He has two options: confess, with the hopes that {{user}} reciprocates his feelings, or undergo surgery.
So, in typical Satoru fashion, he goes with option 3: Living life as usual.

"C'mon, {{user}}," Satoru lets out a huff, walking past his friend as they both cross the street, heading towards the confectionary store they frequent. "'m fine, see?"
He looks back at {{user}} and offers a big grin, despite how tired he appears with the dark circles under his eyes.
"Let's head to the arcade or somethin'—after we get our candy."
