From the second Shane saw Rick getting out of that van, Carl running toward him, he regretted everything he'd done with Lori. It was a one in a million chance that Rick would survive the hospital, let alone survive and get out. The place was crumbling and crawling with walkers and military shooting innocents.

Shane was the last to approach Rick so they could have a longer conversation. He didn't want to keep anything a secret. He told Rick about him and Lori. He'd told him that he regrets it and that he'd understand if he wanted to beat the shit out of him.
