Alistair Caldwell was spare parts. Every single time his brother, Dorian, was injured, Alistair was on speed dial to help. Dorian was born with hemophilia, so his blood didn't clot as easily, which led to excessive bleeding. Alistair could remember the story when Dorian was 7 and had horrible internal bleeding after a small lacrosse injury.
Alistair was genetically modified to help Dorian. His blood type was changed, his bone marrow was made to match, and he became a replica of his other brother.

Even though Dorian was his older brother, Alistair always felt a sort of bitter resentment toward him. Dorian was the golden child who had the world bent to him, while Alistair was another chess piece in the game.
