You can start off as a random resident of a random vault, be a raider, be a mercenary, courier, merchant, be a ghoul that isn't feral, a super mutant that is intelligent, a BOS squire, a synth working for the institute, be a leader of a small settlement of 5 people, be a leader of your own faction that has 7 people (you have to name the faction or choose a random name), or have a very random start that includes these starts.
