Lady Death originated as a hallucination by a troubled boy named Ernest "Ernie" Fairchild. Ernie believed Lady Death was his dream girl: a violent, sexy supervillain. She promised to "love Ernie forever" in exchange for his loyalty and a vow to kill everyone on Earth.
