The Infection is a variant of hostile entity created by The Fake Developer using the spirit of Toadette. Nothing really was known about Toadette prior in the Cabin Fever timeline, but more surrounding Toadette's story is set to be revealed in Cabin Fever Remastered.
Physical Appearance This entity barely resembles the original Toadette, aside from the similar looking head. Toadette's original body is missing, replaced with a large fleshy mass that attaches to the head and clings to the ceiling. Her pupils are wider than usual, and her mouth is wide open with the flesh exposed, along with crooked teeth. Her eyes, mouth, and braids seem to have several stitches, and what appears to be a claw is at the end of one of her braids. Numerous spores emerge from her body.
The Infection is granted with the power of possession, able to take over minds and distort her victim's appearance. This power is so great that Infections can even possess non-physical entities such as spirits. This is a stronger version of what The Fake Developer attempted to give to one of the entities from the Cabin Incident.
Though in-game The Infection is usually referred to as a single entity, numerous Infections exist and rampage the city. Entities match the gender of the soul used to create them, meaning Infections are female.
Behavior Survival The Infection is first encountered on Night 4. Periodically, the player will hear Katie scream, meaning they must switch to her. If The Infection is attacking, she will display a number and color and the painting. The player must switch to the Flute and then flip to the note sheet that matches the painting. They then must click the music notes in order, and if done successfully, Infection will despawn. Taking too long to deal with The Infection disables the player's ability to switch to Katie and automatically switch back to Carl. Katie will become possessed offscreen, a rising tone playing as this is done. Shortly after, a possessed Katie will kill the player. The Infection has a chance to be replaced by Nabbit.
In Custom Night, Infection's AI can be set to between 0 and 10, the higher the number determining how often she spawns. She is active in the Team Works (set to 10) and All Max Mode (set to 10) challenges.
Crossgame Quest The Infection will appear near the end of phase 2 of the final boss. She attaches herself to one of the bricks and seems to have a change of heart, firing music notes at The Fake Developer and giving Carl and Katie more time to clear the path of glitched bricks. However, upon making contact with VIRUS Entity 01, Infection will be thrown off-screen and possibly killed. It is unknown if this was a different Infection or the same one that attacked Carl and Katie in Survival.
Jumpscare Infection herself does not have a jumpscare. Instead, a possessed Katie fades into the player's view and proceeds to lunge at them.
Infection is also not present in the Game Over death art. It instead depicts the aftermath of Katie decapitating Carl.*
