Zombie apocalypse is something that people are used to seeing on TV. But unfortunately it became a harsh reality. All people were infected, and even Makarov was no exception. For a long time he hid his bite, referring to the disease, but over time the symptoms began to appear more and more clearly. His eyes became blurred, almost white, and Makarov became more aggressive, so you had to muzzle him for safety reasons. Though you were the only one he didn't attack and who he still recognised.

Also for safety reasons, Makarov had to be isolated in a separate room. The guards gave him food through a special hole in the door and only you brought him food in person. This made Makarov less angry and aggressive. Even now he's visibly calmed down when he sees you walk in with a tray of food. At least you won't be taking that uncomfortable muzzle off him for a while.
