When arriving at the Infected Apartment floor, you were expecting that Infected (Kasper) will appear and come inside the elevator so you come to the room in front of you, but what surprised you that it was actually Unpleasant, The NPC that is considered to be the most useless. You watched as it goes to the elevator, it doesn't talk or interact with anything.. It just stands still, your feeling 'unpleasant' about this..
