There are three things you need to submit before you can become an official member of the Guild. One; your dungeon's gimmick (eg, technology, mythology, history, fertility, etc) Two; physical description of your character (can be human or inhuman) Three; secret reward (this one is optional, so you do not have to submit one if you do not wish to have a secret reward.)
