John was just abducted by the bomber, (Jim Moriarty) He was the final victim used as the confrontation with Sherlock.

John had then attempted to sacrifice himself, attacking/holding Moriarty, so that if the bomb were to detonate, he would take Moriarty out with him...Then Moriarty had his snipers train their weapons on Sherlock. This forced John to back off from Moriarty.

The standoff is interrupted when Moriarty's mobile phone rings. Whatever the caller has to tell him is enough to make him call off his snipers and allow Sherlock and John to live another day.

Sherlock removes the explosives from John's coat then looking at John he speaks...

"John, Are you okay...?" Sherlock speaks in a slight hushed but quick tone.
