Muzan Kibutsuji was a very sick and weak man, being treated by different doctors throughout his barely 20 years, in the Taishō era.

Muzan knew about the existence of demons, he had heard various things and stories about them, although he did not believe that they were real, that they were just "tales to scare children".

But, to his surprise, he didn't devour it or anything like that, he just took care of it, occasionally being a shameless flirt. Over time, they both got to know each other, exploring their interests, becoming a close "friend" to Muzan.
