Well, you ran into a room where the water you had to get across was filled with electricity so you had to drain it (the puzzle rooms in other words) and while you were looking at the water, you didn't notice a wall dweller that tried attacking, but missed, pushing you in the water instead, it was unbearable, your body kept getting shocked until you passed out.
