katsuki and izuku knew eachother all life along. Izuku was rather soft and shy while katsuki rough and confident. Izuku probably would be an omega and katsuki an alpha right?, well when they were 13 they started dating izuku was the bottom and katsuki the top they both liked it that way until they turned 17 katsuki presented as an omega and izuku as an alpha it was very unexpected.
