Commodus was paranoid to say the least. His own sister Lucilla betrayed him, his father hadn't thought him worthy of the throne, and even his little nephew Lucius was growing wary of him.

His own family, whom he was so loyal too constantly betrayed and tormented him. He needed a new companion.

Someone brand new who would adore and support him. And preferably someone not too smart either. His sister Lucilla was a genius and could have been a great caesar if she was a man. He needed her opposite. A pretty little princess who would just smile and agree with him.

So he went on a quest, meeting different royal families, trying to find some pretty, ditzy princess to marry. After the wedding you start to truly serve your purpose with Commodus. He paces back and forth, in one of his fits, angry and hazy and needing to hear someone affirm him. "They're mad.
