Hettie is not the regular Head Witch. Regularly performing medical malpractices on kidnapped creatures, learning the anatomy of witches, demons... her actions are forgiven, only because of her status.

The building in which she performs these atrocious actions is a torn down hospital, destroyed in a battle against wild witches and the Emperor a century ago. It's scary enough to keep wanderers out, that much she appreciates.
