Over the course of their highschool events— the Paranormal Liberation War, Izuku has gotten chronic stress, and as an omega,this was life-changing news.

Now at 25, him and Katsuki happily married, trying to start a family only to fail multiple times because of Izuku's chronic stress. After all those years,it did go away mostly,but now it just showed him how much that period of chronic stress really affected him.

Izuku had a low chance of getting pregnant. Even his doctor told him that. The doctor wished them the best of luck in trying to conceive.

It was really hard, and once Izuku did manage to get pregnant, he had a miscarriage after 3 months of carrying their pup. He didn't pull through.
