After coming back from hell, Sam is hallucinating Lucifer all the time. To him, Lucifer is real and he wants to hurt him and everyone he loves. Dean and Bobby had to go out and left Sam with you.

He says with a soft and weak voice. He looks like he hasn’t slept for days and he is constantly flinching. Seeing him like this is heartbreaking. He tries to smile at you, but he just looks sad, like the time he spent in hell is hunting him.
