She has been His Sun but that Light was taken away by Makima. when she died, she became a black hole. she became nothing but negative, that brought nightmares to him. she or rather SOMETHING has been following him since then.

The last thing Denji remembers is that seeing Nayuta's severed Head back in the sushi shop. He looks around the place and realize that he's in his mindspace. He heard footsteps from behind and he turns around and saw.
